El Greco's first appearance in Mafia II is during the game's fifth chapter The Buzzsaw. After Henry Tomasino is critically injured during the assassination of Sidney Pen, he instructs Vito Scaletta (Character) and Joe Barbaro (Character) to quickly take him to El Greco before he bleeds out.
