    The player characters encounter Al at the very beginning of the Flashback scenario. He at first functions as a guide to help the players find their way through the forest to the abandoned hospital. The zombies that patrol the forest will pay him no need, however.

    Upon arriving at the hospital it's revealed that Al is in fact insane and has been luring people to the hospital so he can feed them to a giant plant. The plant had devoured his infected wife, thusly resulting in Al believing that the plant is his wife.

    Upon exploring the hospital, Al will routinely show up in his Axe Man garb to chase and attack the players. He cannot be killed, but can at least be damaged enough so he'll temporarily flee. The axe he wields is extremely powerful and can inflict bleeding as well; one of his attacks is an instant-kill. Though it requires a short windup to execute, thereby giving players the chance to escape from its range. His axe can eventually be procured, and, unlike other melee weapons, does not degrade; it is only available on the Easy and Normal difficulty settings, however.

    Once the giant plant has been killed, the corpse of Al's wife, Dorothy, will seep out of the creature. As the plant was keeping much of the building's structural integrity intact, the hospital then begins to collapse on top of the two, burying them beneath the rubble.
