Sometimes designers add old-school things on purpose to enhance game design. These games tend to be heavily inspired by hardware limitations of older systems. NES, Atari 2600, and early computer platforms (DOS, Commodore 64, MSX, etc...) are common sources of inspiration.
Lenticulars are images that change or move when viewed from different angles. Occasionally they are used on the cover of a game. The process is based on stereoscopic vision and is made possible through the use of arrays of carefully designed parallel lenses.
Some games replace letters or words in a title with numbers or symbols, usually to add more of a "badass edge" to them.
Whether it's for minimalism or a lack of artistic effort a lot of games use only the games logo as it's boxart. Examples would include all Final Fantasy's after VII in PAL and Japanese territories, Gran Turismo 3, Portal, Turok 2: Seeds of Evil, Metal Gear Solid and more!
Games that include real photography on the game's cover.
Showing half a face on box art is often used to give a sense of power or mystery.
Games that for some reason or another, include a slip cover. Usually Collectors Editions or first run games.
