In the 2004 Punisher video game is not Billy Russo, who is Jigsaw in the comics and show, instead, he is John Saint. This was done to tie the video game to the 2004 Punisher movie. John Saint in the movie is the son of Howard Saint and he is sent by his father to kill the entire Castle family and "avenge" his brother Bobby. Frank Castle survived and became the Punisher and went on to kill the Saints. When Frank confronts John, John is in an office with a shelf crushing his right arm and reaching for a gun with his left hand. The Punisher moves the gun away and places a bomb in his hand with a string tied to the pin. The Punisher wraps the string around dangling debris and has John hold the bomb up to keep the pin from coming out if he wants to live. However, John's arm gets tired and the bomb goes off and explodes on John. It was presumed in the film that John died when the Punisher kills Howard Saint. However, the game reveals that wasn't the case.

In the Punisher video game, Jigsaw attempts to run over the Punisher with a car. The Punisher traces the license place to a chop shop owned by the Gnucci crime family and raids the chop shop. This sends the Punisher on a warpath against the Gnuccis, the Russians, and eventually the Kingpin. When the Punisher confronts the Kingpin at Fisk Tower, the Kingpin reveals to the Punisher that Frank was duped into going after him and weakening Fisk's criminal empire. After the raid of Fisk Tower, the Punisher meets an informant who tells him that the Kingpin was telling the truth and that Jigsaw was using the Punisher to take down Fisk. The Punisher learns that the Jigsaw rose through the ranks of a Yakuza gang called the Eternal Sun and that he wanted to run New York's crime scene and kill the Punisher. The Punisher goes on to thwart Jigsaw's efforts to take over old Gnucci and Russian criminal operations. After that, the Punisher learns about Jigsaw's plan to hit Stark Industries and steel Iron Man technology from Stark. The Punisher goes to Stark Industries in an attempt to get Jigsaw and learn about him. Jigsaw gets away with Stark technology. However, after the attack on Stark Tower, the Punisher learns that Jigsaw is John Saint and that he survived the explosion that scarred his face. The Punisher also learns that Jigsaw planned on ousting the Eternal Sun's leader Takagi. The Punisher goes to the nightclub that is run by the Eternal Sun and kills Takagi. Before he kills Takagi, the Punisher learns that Jigsaw is on Ryker's Island and is planning on escaping. After the Punisher kills Takagi, he turns himself over to the authorities to finish Jigsaw. Jigsaw orchestrates a prison riot and plans to escape in the confusion. Jigsaw assembles his Iron Man suit and uses it to try and kill the Punisher. However, the Punisher destroys the jetpack on Jigsaw's suit and uses landmines to shut the suit down. Then the Punisher drops Jigsaw from a helicopter into a police van, killing him.
